Before you can start capturing data, you need to add at least one form to your project. Once you have logged into the console, you can add a new form at any time.

To add a new form:

  1. Open the quick-switch panel and select the project you want to add the form to.
  2. Select '+ New Form'.

Once you have selected '+ New Form', a dialog box will open and you will have two options:

  1. New: add a blank form and create its content from scratch
  2. Duplicate: duplicate a form that already exists in this project (or in another project you have access to)

Adding a blank form

To add a blank form and create your form's content from scratch:

  1. Select 'New' from the dialog box.
  2. Add the details of the form:
    • Form Name: this is the name that you will see on the console and that your fieldworkers will see on their handsets. You will be able to change this name at any stage during the project from the form's Design page.
    • Default Language: this is the language in which the form will be created. The form's default language cannot be updated once the form is created, but you can add translations later.
  3. Select 'Create Form'.

A blue banner will appear at the top of the page confirming that you have successfully created the form and the Design page of the new form will open. You can now start designing your new form's content.

Note: Your new form will have a Draft status until you Publish it.

Alternatively, you can navigate to the Dashboard of your selected project, and select '+ New Form' on the ribbon:

Duplicate an existing form

To duplicate a form that already exists in this project or in another project you have access to:

  1. Select 'Duplicate' from the dialog box.
  2. Choose the form you want to duplicate:
    • Select Project: select the project containing the form you wish to duplicate from a drop-down list of all the projects you have access to.
    • Select Form: select the form you wish to duplicate from a drop-down list containing all the forms in the project you selected above.
    • Form Name: this is the new form name that you will see on the console and that your fieldworkers will see on their handsets. This will default to the same name as the form you are duplicating, appended by "(copy)". You can choose another name and will also be able to update it at any stage during the project from the form's Design page
  3. Select 'Duplicate Form'.

A blue banner will appear at the top of the page confirming that you have successfully duplicated the selected form, and the Design page of the duplicated form will load. This form will be exactly the same as the original form, including all groups, fields and logic.

Note: Your new form will have a Draft status until you Publish it.

Alternatively, you can navigate to the Dashboard of your selected project, click on the cog next to the form you want to duplicate, and select 'Duplicate' from the drop-down menu. This will directly add a copy of your form and list it under Draft forms on your Dashboard.
