Before you can start capturing data, you need to add at least one form to your project. Once you have logged into the console, you can add a form at any time.


To add a new form:

  1. Open the quick-switch panel and select the project you want to add the form to. 
  2. Select '+ New Form'.





Alternatively, you can navigate to the Dashboard of your selected project, and select '+ New Form' on the far right of the ribbon:





Once you have selected '+ New Form', a dialog box will open and you will have three options:

  1. New: add a blank form and create its content from scratch
  2. Duplicate: duplicate a form that already exists in this project (or in another project you are an administrator of)
  3. Import: import a form from your hard drive


Adding a blank form

To add a blank form and create your form's content from scratch:

  1. Select 'New' from the dialog box.
  2. Add the details of the form:
    • Form Name: this is the form name you will see on the console. You will be able to change this name at any stage during the project.
    • Form Title: this is the form name that the fieldworkers will see on their handsets. You will be able to change this name at any stage during the project.
    • Default Language: this is the language in which the form will be created. 
  3. Select 'Create Form'.


A green pop-up will appear at the top of the page confirming that you successfully added a form. The Overview page of the new survey will open. You can now start designing your new form's content.



Note: Your new form will have a Draft status until you change the status to Live.





Duplicate an existing form

To duplicate a form that already exists in this project or in another project you are an administrator of:

  1. Select 'Duplicate' from the dialog box.
  2. Choose the form you want to duplicate:
    • Select Project: select the project containing the form you wish to duplicate from a drop-down list of all the projects you have access to. 
    • Select Form: select the form you wish to duplicate from a drop-down list containing all the forms in the project you selected above.
    • Form Name: this is the form name you will see on the console (internal identifier). This will default to the same name as the form you are duplicating, but you can choose another name. You will be able to change this name at any stage during the project. 
    • Form Title: this is the form name that the fieldworkers will see on their handsets (external identifier). This will default to the same title as the form you are duplicating, but you can choose another name. You will be able to change this name at any stage during the project. 
  3. Select 'Duplicate Form'


A notification will appear at the top of the page,and the Overview page of the duplicated form will load. The form will be exactly the same as the original form, including all sections, questions and logic.



Note: Your duplicated form will have a Draft status until you change the status to Live.
